Application Form to Subscribe to the GS1 System

A company which wish to subscribe to the GS1 System must submit the following:

1. For Category A, Category B & Category C, the completed Application Form acrobat file, 1,908.0 kb) and List of countries for export market (PDF file, 72.4 KB).
   
   (Please read the Rules and Regulations Governing Subscribers to the GS1 System)

2. Certified true copy of the following document(s):
   • For Sole-Proprietor/Partnership - Peninsular Malaysia and Sarawak
     1. Business Registration Certificate
     
     
   • For Sole-Proprietor/Partnership - Sabah
     1. Licence to trade
        
     2. Photocopy of the identity card(s) of the sole proprietor / partners
     
     
   • For companies registered with the Company Commission of Malaysia (formerly Registrar of Companies:-
     1. Certificate of Incorporation
        
     2. Latest Form 49
        
     3. Latest Form 24
        
     4. Latest Annual Returns
     
     
   • For Government / Statutory Bodies:
     1. Letter from the Secretary-General or a senior level of management informing that the organisation wishes to subscribe to the GS1 System
        
     2. Annual Report on the activities of the organisation
        
     3. Working capital / annual budget, if any

The processing period is seven working days after receipt of complete and correct information, payment and documentation.
